When should I get my pneumococcal vaccine?
This vaccine is available year-round for anyone 65 or older. If under the age of 65, a prover order is needed.

How can I get a copy of my child’s immunization records?
Call ahead a paper copy will be available for pick-up at the front window. OR, We will email you a form from My Vax Indiana, with simple instructions and a pin number to be used to get to your child’s immunization record. Keep this pin number to use anytime to see updates.
